Such a child is under close attention of medical personnel throughout his studies. The school register is kept by the nurse.

How to draw up a document

The format of the inspection log depends on the form of maintenance. The document is kept in paper form and electronically. All data is entered into a specially developed program. Reports are generated in it, and the table is printed. You can separately select sick children and a list of activities carried out. In the absence of a computerized program, all information is stored in a paper inspection log. According to , doctors keep a journal in kindergarten for several years, or start a new document every year.

Pediculosis is one of the diseases that is easier to prevent. A timely examination allows you to detect the problem in time, take appropriate measures, and avoid problems with the child’s health and psyche. The main symptoms of the disease are severe itching, scratching, and scratching on the head. Preschool children and primary school students are at risk. The form, the journal, helps to classify patients and determine the degree of prevalence.

The high level of infectious diseases in preschool children is associated with the immaturity of their immune system and the many viruses that cause infections in the environment.

Most often, preschoolers become infected with diseases transmitted by airborne droplets (by coughing, sneezing), and less often by diseases transmitted by direct contact (through shared toys, handkerchiefs, cutlery, etc.).

Quarantine(Italian quarantena, from quaranta giorni - forty days) - a system of measures taken to prevent the spread of infectious diseases from an epidemic outbreak and to eliminate the outbreak itself.

The epidemic process of these diseases is characterized by seasonal changes, outbreaks and emerging epidemics of varying intensity, which affect about 20% of children.

If an infectious disease (flu) occurs in a kindergarten, the group attended by the sick child is quarantined.

Today, quarantine periods are set based on data on the longest incubation period of the disease. Thus, for influenza and scarlet fever, quarantine lasts 7 days, for chicken pox, rubella and mumps - 21 days, for meningococcal infection - 10 days, and for viral meningitis - 20 days.

Responsibility for maintaining quarantine in the kindergarten rests with the manager.

Let's take a closer look at the activities of the head of a kindergarten during quarantine due to the flu using the example of the MDOU "Child Development Center - Kindergarten No. 000" (hereinafter referred to as the DOU).

Documentation developed and used during the quarantine period

In order to ensure sanitary and epidemiological well-being, prevent the occurrence and spread of an infectious disease in a preschool educational institution, on the day of registration of the disease, the head issues an order “On urgent measures to prevent the spread of an infectious disease (influenza) in a preschool institution” (hereinafter referred to as the Order; Appendix 1).

Simultaneously with the publication of the Order based on sanitary and epidemiological rules and regulations "Sanitary and epidemiological requirements for the design, maintenance and organization of the operating mode of preschool educational institutions. SanPiN 2.4.1.1249‑03", approved. The Chief State Sanitary Doctor of the Russian Federation on March 25, 2003 (hereinafter referred to as SanPiN), an infectious disease doctor and a senior nurse develop:

· wet cleaning schedule (Appendix 2);

· room ventilation schedule (Appendix 3);

· quartzization schedule (Appendix 4);

· quarantine briefing log (Appendix 5);

· plan of anti-epidemic measures during the period of rising incidence of influenza, ODS (Appendix 6);

· a memo on the prevention of infectious diseases (influenza) (hereinafter referred to as the Memo; Appendix 7);

· medical observation sheet for contact children (Appendix 8);

· examination log of contact children (Appendix 9);

· log of preventive measures (Appendix 10);

· log of transfer of contact children (Appendix 11).

All documents are approved by the head of the preschool educational institution.

In addition, the head nurse is required to maintain the following documentation during the quarantine period:

· bactericidal installation log;

· journal of infectious diseases.

Medical personnel place a reminder on the prevention of infectious diseases (influenza) on the information stand. According to this document, group teachers conduct consultations, meetings and conversations with parents of pupils.

Conducting an unscheduled administrative council

After the Order is issued, the head of the preschool educational institution organizes an unscheduled administrative council (hereinafter referred to as the Council), which includes: deputy heads for water management and administrative affairs, medical personnel, as well as teachers and a junior teacher of the quarantine group.

At a meeting of the Council, the manager brings the Order to the attention of employees, and the senior nurse instructs on the observance of preventive measures and reports:

· quarantine periods;

· schedules for the disinfection regime in the group, ventilation and quartz treatment in the quarantined group;

· features of the drinking regime;

· timing of final disinfection;

· peculiarities of removing children who have not been in contact with sick pupils.

After the briefing, all present employees sign in the quarantine briefing log.

According to the Order, control over the implementation of anti-epidemic measures is carried out by the deputy head of administrative affairs and the senior nurse.

The deputy head of the administrative department and the senior nurse should pay special attention to conducting daily rounds in order to monitor compliance with the disinfection regime.

The results of control are reported at administrative councils.

Monitoring the health status of children visiting the preschool medical center

During the quarantine period, the head organizes monitoring of the health status of children who come to the preschool medical center with signs of an infectious disease.

Monitoring is carried out according to completed documents: infectious diseases register; medical observation sheet for contact children; a log of examinations of contact children, where the results of the first and final examinations are entered; a register of transmission of contact children, where medical personnel register each case of the disease in order to transfer information to the medical organization for further action.

RECOMMENDATIONS FOR REGISTRATION AND MAINTENANCE OF GROUP DOCUMENTATION IN THE MUNICIPAL PRESCHOOL EDUCATIONAL INSTITUTION "KINDERGARTEN No. 99 OF GENERAL DEVELOPMENTAL KIND" OF SYKTYVKARA. Methodological recommendations for preschool teachers. Syktyvkar, 2010

The collection includes methodological recommendations for teaching staff of preschool institutions on the preparation and maintenance of group documentation in kindergarten. A list of documentation is determined, forms of group documentation are presented, and some methodological recommendations are given for the preparation and maintenance of group documentation.

Introduction

The success of any activity largely depends on its proper organization. Educational activities in a preschool institution are also no exception.

We believe that one of the ways to fully and efficiently organize the educational process is for teachers to maintain the necessary group documentation. Information about the families of the pupils, activity planning, information about the state of health, etc. contribute to the creation of the necessary conditions for organizing a rational process of upbringing and teaching preschool children.

The process of preparing and maintaining group documentation requires mental effort and a huge amount of time from the teacher. The question arises: “How to organize this process without interfering with educational activities?”

We found a way out - to develop and distribute printed group documentation forms (diagrams, tables, etc.) for teachers. This will help reduce time costs and facilitate the process of maintaining group documentation.

In this manual, we presented a list of group documentation for the municipal autonomous preschool educational institution “Kindergarten No. 99 of a general developmental type” in Syktyvkar, methodological recommendations for maintaining each component of group documentation, as well as samples of group documentation.

List of group documentation in preschool educational institutions

Our preschool has determined a list of group documentation that must be completed and maintained in each age group of the kindergarten.

In accordance with the list approved by order by the head of the preschool institution, teachers are responsible for preparing and maintaining the following documentation:

1. In all age groups of preschool institution:

- “Attendance”,

- “Social passport of families of pupils”,

- “Hardening”,

- “Payment for kindergarten”,

- “Payment for additional services”,

- “Protocols of parent meetings”,

- “Quarantine notebook”,

- “Perspective-thematic planning”,

- “Calendar-thematic planning”,

- “Notebook of health”,

- "Comments and suggestions",

- “Interaction with specialists”,

- “Planners”,

- “Learn with us.”

2. In early age groups and in the first junior group (from 1.5 to 3 years) the following are added:

- "Morning reception"

- “Chair map.”

All components of group documentation (with the exception of “Feedback and Suggestions”, “Planning Meetings”, “Learn with Us”, “Interaction with Specialists”) are issued in the form of magazines on a printed basis in A4 format.

General requirements:

Group documentation is stored in preschool groups in a specially designated place (preferably on the teachers’ desk),

Entries in documentation are made in a timely, accurate and legible manner,

Teachers are personally responsible for the maintenance and aesthetic appearance of group documentation,

At the request of the administration, educators must provide for verification all components of group documentation,

Twice during the academic year, group documentation is subject to mandatory control by the administration.

for registration and maintenance of group documentation
1. "Attendance"- this is a group documentation document, which is an A 4 format journal on a printed basis, allowing you to track the attendance of kindergarten students, as well as the reasons for the absence (passes) of children.

The form of the student attendance monitoring log is presented in Appendix No. 1.

Records must be kept in a timely, legible and accurate manner,

In the table, weekends are allocated for each month (using a pencil or colored pens),

The notebook is filled out daily after the morning reception of children as follows: in the cells with the letter “n” children who did not come to kindergarten that day are marked, the remaining cells remain empty,

During the day, the reason for the child’s absence is determined by telephone and indicated in the journal on the corresponding page in the table - on the back of each month; thus, the reason for each child’s absence from kindergarten is noted,

At the end of the list, the attendance of the group teachers and their work is noted (on those days when the teacher was absent, put the letter “n”; when he worked in 2 shifts, put the number 2).

2. “Social passport of families of pupils” - This is a group documentation document, which is an A 4 format journal on a printed basis, allowing you to collect information about the family, housing and social situation of the families of the pupils, as well as the place of residence and contact numbers of the parents.

The form of the journal “Social Passport of Families of Pupils” is presented in Appendix No. 2.

The social passport contains information about the families of all pupils in the group (based on the payroll),

All columns in the table are filled in, without exception: the first column contains the student’s last name, first name, patronymic, date of birth, medical policy number and birth certificate number; the second column reflects information about the place of residence of the student’s family (city, street, house number, apartment number, home telephone number, type of housing, number of rooms in housing); the third and fourth columns reflect information about the mother and father separately (last name, first name, patronymic, place of work and position, date of birth, education, contact numbers (cell and work), number of children in the family), information is entered in the last column about the pupil’s close relatives (for example, grandmother – last name, first name, patronymic, contact phone number, etc.),

Entries in the social passport must be made accurately, legibly and in a timely manner,

If there is no information (no home phone number, one of the parents is missing, etc.), then a dash is placed in the corresponding column,

The social passport must contain reliable information and be updated annually, as well as as necessary (if any data changes).

3. “Hardening” - this is a group documentation document, which is an A 4 format journal on a printed basis, allowing you to reflect information about the implementation of hardening procedures in the group.

The form of the journal “Hardening” is presented in Appendix No. 3.

Requirements for registration and maintenance:

The journal is drawn up daily by the group’s teachers after hardening procedures,

Hardening is carried out according to the Speransky system (based on the principle of air contrasts), so it is necessary to monitor temperature conditions: before and during daytime sleep, the air temperature in the group is set to 19°, while in the bedroom, with the help of heating devices, the air temperature rises to 25°. At the end of daytime sleep, children in shorts and T-shirts run from one room for 5-8 minutes to another along the “health paths” (children of preparatory groups 6 times, older ones - 5 times, middle ones - 4 times, second youngest - 3 times, first juniors - 2 times), thereby being exposed to contrasting air influences. Once a week the temperature is reduced by 1°. Thus, the temperature gradually decreases in the preparatory and senior groups to 13°, in the middle and junior groups - to 15°. Having reached the maximum mark, the procedure begins again (from 19◦).

Entries in the journal are made carefully, legibly, in a timely manner as follows: “n” - was not in kindergarten, I - healthy child, II - gentle regime (children are divided into 2 groups: 1 - healthy children, 2 - frequently ill and those who came after an illness ). For frequently ill children and children after illnesses, establish a gentle regime - corrective gymnastics is carried out in the bedroom. After an illness, the child does corrective gymnastics for 5-7 days together with the 2nd group of children. If the child has been ill for a long time and with complications, then the gentle regime is extended to 2-3 weeks.

Sewing-adhesive bonding ? Sewing binding

(makes binding more durable)

Sewing binding During the manufacturing process, printed sheets are used that are folded, assembled with notebooks and stitched together with threads, which gives the block high strength. Next, this block is inserted into the binding cover. (One binding cover is used, consisting of 2.5 mm cardboard covered with paper vinyl). Then the block is connected to the lid using a flyleaf - a sheet of paper. A captal is glued to the upper and lower parts of the spine of the block for visual beauty and completeness of the product.

Laminate the cover: ?
Lamination- This is the coating of printed products with film. Lamination will preserve the attractive appearance of printed products for a long time and reliably protect them from contamination and mechanical damage. We carry out single-sided and double-sided lamination up to A1 format using special devices - laminators.

The main purpose of hot lamination is to protect the image from various external influences, which may include spilled coffee, all kinds of attempts to wrinkle, scrape, scratch the image, wet cleaning of premises, rain, snow. But experienced users also know about another valuable property of lamination: it can significantly improve image quality. When using glossy films, the image “appears” and the colors become more contrasting and richer. The "develop" effect gives inexpensive laminated paper the appearance of luxurious photo paper.

Preschoolers most often become infected from each other with diseases transmitted by airborne droplets (by coughing, sneezing), and less often by diseases transmitted through close contact (through shared toys, mugs, spoons, plates, handkerchiefs, etc.). To exclude such situations, a quarantine is introduced in a preschool institution with information about deadlines and events recorded in a log.

Quarantine in a preschool educational institution: order, log, events, deadlines

The epidemic process of diseases is characterized by seasonal changes, outbreaks and emerging epidemics of varying intensity, which in general affect about 20% of pupils. If an infectious disease (flu) occurs in a preschool institution, then the timing of the event in the preschool educational institution is reflected in the order and journal for the group that the sick child attended and is in. The main purpose of quarantine in a preschool educational institution is to prevent the spread of the epidemic among kindergarten students.

The duration of quarantine is established based on data on the longest incubation period of the disease. For example, quarantine for scarlet fever in kindergarten or for the flu lasts 7 days, quarantine for chicken pox, mumps, rubella - 21 days, for meningococcal infection - 10 days, and for viral meningitis - 20 days.
